Cal/OSHA Fines Fabricator $212K Over Employee Finger Amputation

California safety regulators levied a heavy fine against a metal fabricator over an incident that cost a worker a finger.

Cal/OSHA fined All FAB Precision Sheetmetal $212,850 after a June 2025 press brake incident in San Jose amputated a worker’s finger. All FAB Precision Sheetmetal is a San Jose sheet‑metal company previously cited in connection with a similar 2024 amputation involving the same type of equipment, according to Cal/OSHA.

The agency issued one willful regulatory violation, one serious accident‑related violation and one willful repeat serious accident‑related violation. Investigators said the company failed to correct a known machine‑guarding hazard and continued to allow operation of the press brake without effective safeguards, despite a prior incident and regulatory requirements.

The case was referred to Cal/OSHA’s Bureau of Investigations for potential criminal review. The employer has filed an appeal with the Occupational Safety and Health Appeals Board. Machine‑guarding failures remain a leading cause of amputations and crushing injuries, particularly when employers continue operating equipment without required protections, according to Cal/OSHA.
